VITAL STATISTICS. POPULATION. The population of the Borough at the Census of 1911 was 133,487. The estimated population, based on rationing statistics at the middle of 1917, was 131,008. It is necessary for the present to continue the use of the estimate of population as in 1917, because that for 1918, although it might now be used, would necessarily differ more from the facts of tihe present time than the earlier estimate. The main change which occurred between mid-1917 and mid-1918 was further depletion of the male civilian population by enlistment, and as this has now been more than made good by demobilisation, it follows that tihe 1917 estimate, though it understates the present population, does so to a less extent than would that for 1918. BIRTHS. During fhe year 2,205 births were registered (males 1,149, females 1,056) as against 2,634 in 1917, giving a birth-rate oi 15"3 per 1,000 population. Illegitimate Births. Forty-nine of these were registered. DEATHS. The number of deaths during tihe year was 1,943, of which 201 were of persons of 65 years and upwards. 458 deaths were reported of persons belonging to the Borough which had taken place in Institutions outside the district. 7 Including these we have a nett total of 1,943, which gives a rate of 15.1. The death-rate for England and Wales was 17.6, and for the 96 great towns 18.2. INQUESTS. 83 inquests were 'held, 55 dearths being attributed to violence. INFANTILE MORTALITY. 154 deaths of infants under 1 year of age occurred in the Borough, giving a rate of 69 per 1,000 births registered as compared with 85 per 1,000 in 1917. No. of notified Births 1,956 No. of unnotified Births 249 No. of Deaths under 1 month 50 No. of Deaths under 1 year 154 A total of 154 children died under one year, giving an infant mortality rate of 69 per 1,000. The infantile mortality rate in the 96 large towns of England and Wales was 97 per 1,000 births. The Health Visitors have paid in all 6,808 visits to houses where births have been notified, 4,580 of which have been second 8 or third visits ; 139 visits were paid for special purposes—in cases of illness, at the request of parents, etc. 71 ante natal visits were paid, with 65 re-visits. During- the measles epidemic 642 first visits and 213 re-visits were paid by Miss Buckton, who was detached from infant visiting- for t'his purpose. Infant Consultations have been 'held twice weekly at the old Council Offices, and the number of attendances (5,197) made testifies to their usefulness. The Council having adopted my recommendation by appointing a Lady Assistant Medical Officer for the Maternity and Child Welfare department, two more Health Visitors, and more suitable premises for the Ante Natal and Infantile Consultation will further help to deal with the prdblem of preventible infant mortality. MIDWIVES' ACT. The number of Midwives giving- notice of their intention to practice in the Borough during the year wes 60.41 of these were attached to the Plaisitow Maternity Charity, 6 to Sir Henry Tate's Nurses' Home at Silvertown, and 13 were in private practice. Of these 2 left the neighbourhood during the year, 1 had no cases, 2 others 1 and 3 cases respectively. 42 visits were paid by the Inspector of Midwives for the purpose of inspection of bags and records. The nurses working at the Maternity Home at Plaistow did not come under the Midwives' Act for the first half of the year, as during that time a resident Medical Officer was appointed. (See Section E., 26.) 870 cases have been taken by doctors, 992 by midwives. There have been 59 still-births notified. Of these 33 were doctors' cases, 26 midwives. Midwives have notified three cases of Ophthalmia. These have been visited and kept under observation unitil well. One case of puerperal fever has been notified in the Borough during the year. 9 120 records of sending' for medical help have been received. For the Mother:— Malpresentations. Prolonged Labour. Dis. from Eyes. Ophthalmia Other Causes 19 2 7 3 7 NOTIFIABLE INFECTIOUS DISEASES. 2,173 cases of Infectious Disease were notified during the year. Small Pox. Three cases of this disease occurred, but all contacts were from one case. No death occurred. Scarlet Fever. 272 cases of this disease were notified during the year as against 341 in 1917. Two deaths occurred from the disease. 237 cases were removed to the Isolation Hospital. Diphtheria. 273 cases of this disease were notified, with 18 deaths. 279 cases were notified in 1917, with 20 deaths. 251 cases were removed to the Isolation Hospital. Enteric Fever. Two cases of Enteric (Typhoid) Fever were notified, with 2 deaths. Erysipelas. 52 cases of Erysipelas were notified as against GG cases in 1917. There was 1 death. 10 Ophthalmia Neonatorum. During the year 11 cases were notified. Puerperal Fever. One case of this disease was notified as against 2 cases in 1917. Epidemic Cerebro-Spinal Meningitis. Three cases of this disease (commonly known as Spotted Fever) were notified, all of which proved fatal. Acute Polio-Myelitis. There were no cases. Typhus Fever. No case was notified. Measles and German Measles. 1,519 cases were notified, with 28 deaths. Tuberculosis. The number of deaths from Tuberculosis wias 197, as compared with 229 in 1917. The following table shows the number of deaths from Pulmonary and otheir forms of Tuberculosis :— Pulmonary Tuberculosis (Phthisis) 165 deaths Tuberculosis of the Brain (Meningitis) 13 deaths Other forms of Tuberculosis 19 deaths Under the Public Health (Tuberculosis) Regulations the total number of notifications received was Tuberculosis (Pulmonary) Tuberculosis (Other forms) 283 as against 251 in 1917 80 as against 81 in 1917 363 332 COMMUNICABLE DISEASES—NON-NOTIFIABLE. Whooping Cough. 21 deaths were registered as due to Whooping Cough, as against 18 in 1917. Acute Encephalitis Lethargica. During the early part of the year 8 cases of the above rare disease came under my observation. Four deaths occurred. 11 Influenza. During the last months the world-wide epidemic of Influenza gave considerable anxiety. Owing to the severity of the disease and complications, many deaths occurred in this Borough. During the year the number of deaths due to this disease and complications was 491. Great difficulty was experienced, owing to the shortage of labour, to obtain the necessary undertakers and grave diggers for the disposal of the bodies, but this was eventually overcome by the bodies being removed to the mortuaries to await interment. The Medical Practitioners of the distriot were unable to cope with the work in connection with the epidemic, and I applied to the War Office for the release of several who were in the R.A.M.C., but I was only able to obtain the assistance of Dr. Brough, Deputy Medical Officer oif Health, for 14 days during the height of the epidemic. Two empty wards at the Isolation Hospital were opened for the admission of cases with complications, but the results were not altogether satisfactory, owing to the severe complications oif the cases admitted. REPORT ON THE BOROUGH BACTERIOLOGICAL LABORATORY. During the year 1918 there were 310 specimens examined for Medical Practitioners. Nutrition. 13.1 boys and 116 girls, percentages of 10.9 of boys and 11.7 of girls were found under normal conditions. Defective Clothing. Ragged or insufficient clothing was found in 1.4 per cent. boys and 0.50 per cent. girls. Dirty clothing was found in 1.3 per cent. boys and 1.5 per cent. girls. Defective Boots. There was noted a percentage of 4.6 boys and 2.8 girls. External Eye Disease. These diseases are much more rare than in former years as cases are discovered early and treated at the Clinics. 17 boys and 32 girls were found affected. Skin Diseases. Impetigo. The teachers send children who are infected to the Clinics as soon as detected. The number of cases found at routine inspections was 29. 30 Bingworm. During the year 77 cases of ringworm have been referred to the London Hospital for X-ray treatment. This treatment is much more satisfactory than the treatment by local applications as previously carried out. Scabies. This troublesome complaint has been checked by the frequent bathing at the Cleansing Station. Five cases were found at inspections. Disease of Nose and Throat. Slight enlargement of tonsils existed in 309 boys, a percentage of 25.9, and 240 girls, a percentage of 25'2. Greater enlargement, requiring surgical treatment, existed in 110 boys and 102 girls. Adenoids are frequently associated with enlarged tonsils. Arrangements have now been made for treatment to be carried out at a Hospital, the Committee being responsible for the cost in necessitous cases. Deafness. This existed in 25 boys and 23 girls. This disease was most frequently associated with discharging ears, adenoids and enlarged tonsils. Disease of Heart and Circulation. Heart affections were present in 82 boys and 68 girls, percentages of 6.8 in each case. Disease of the Lungs. Affections of the lungs were present in 36 boys and 52 girls. Those who had suspicious physical signs have been referred to the Tuberculosis Dispensary and kept under observation and treated on Tuesday afternoons. Defective Mental Condition. The number of boys was 106 and girls 83 found below average intelligence. The special classes at Sandringham Road and Shrewsbury Road Schools, under teachers specially trained in the teaching of backward or mentally deficient children, are still in existence and doing good work, but are insufficient for the TABLE III. The report of the Dentist is appended. The demand for dental treatment has considerably increased, and the time has now arrived when the Committee should take into consideration the appointment of a full-time Dentist. Parents are now beginning to realise the importance of having their children's teeth attended to, and we now have a large number waiting for treatment. Cleanliness. All the school children have been examined by the School Nurses and the results are shown in Table VII. Out of a total of 37,742 examinations 2,403 were discovered with a few nits in their hair, 504 with many nits and presence of vermin and 64 with body and clothing verminous. Report OF THE Chief Sanitary & food Inspector FOR THE YEAR 1918. 42 Annual Report of the Chief Sanitary and Food Inspector. To His Worship the Mayor, the Aldermen and Councillors of the County Borough of East Ham. Mr. Mayor and Gentlemen, I have the honour to submit to you my twenty-third Annual Report on the work done by the Sanitary Inspectors during the year ended 31st December, 1918. The total number of inspections made during the year under consideration was 1,962. These include visits to premises upon complaints of nuisance, inspections of markets, workshops, slaughterhouses, bakehouses, cowsheds, dairies, etc. There have been 2,009 nuisances discovered, comprising choked and defective drains, leaky roofs, defective sanitary appliances, animals improperly kept, etc. In obtaining the abatement of the aforesaid nuisances, the following Notices were served:— Preliminary 844 Statutory 24 Dust Bin 141 Water 7 making a total of 1,016 Notices. During the period of the War this work had to be partially suspended, but it is hoped that normal business conditions will soon prevail and this very important branch of Public Health service will be resumed ; for it is within our knowledge that the smaller houses in the Borough are seriously in need of repair, and with the present Staff it will take considerable time to overtake the arrears of work. 44 DIRTY PREMISES. Under this heading are included dirty and dilapidated walls and ceilings, etc. For the past four years Notices have only been served in cases where the conditions are such as to be a serious nuisance. Many occupiers have realized the situation and accepted the inevitable, while some have whitewashed the ceilings and cleansed the walls of their houses themselves. Very much work now remains to be done, but the excessively high prices of material form a formidable obstacle. It is difficult to persuade people who have always lived in rooms where the walls have always been papered, that distempered coloured walls are healthier and more easily obtained. ROOFS, RAINWATER PIPES AND EAVES-GUTTERS. Neglect to keep the roof, gutters and rainwater pipes in repair soon causes serious damage to property, and to the comfort and health of the occupants. Owing to circumstances over which we have had no. control, eaves-gutters and rainwater pipes have been most difficult to obtain, and properties have suffered considerably. Defective eaves-gutters are the most common form of nuisance. They are easily detected, but most consistently neglected by the owners and their agents. In two instances it was necessary to take legal proceedings to compel owners to repair the roofs and gutters of their own houses, as the rainwater was saturating the premises to such an extent as to render them uninhabitable. DRAINS UNSTOPPED AND CLEANSED. The work of unstopping and cleansing drains has received the usual attention. At 1,344 houses the drains have been unstopped and cleansed, being 4.9 per cent. of the houses in the Borough. 45 DRAINAGE WORK. At 405 houses drains have been tested, and in twelve instances defects in the drains have been discovered, all by routine work, such as visits caused by the occurrence of a case of infectious disease, or complaint of offensive smells. In addition to the twelve defective drains there were eighteen defective water-closet connections and 17 defective water-closet traps. In 37 instances soil pans were renewed or cleansed, and 28 new ventilating pipes were erected. For the carrying out of these sanitary amendments the District Inspectors supervise and pass the work. No pipes were permitted to be covered until they were tested. SANITARY APPLIANCES. This term includes the appurtenances to the drains proper and during the year there have been 110 apparatuses to the water closets .renewed or repaired, and 48 waste pipes from scullery sinks and baths were dealt with. DISEASED AND UNSOUND FOOD. The markets, slaughterhouses, and premises used for the preparation and sale of food have received a considerable amount of attention. In the early months of the year we examined and dealt with a large quantity of meat, bacon and other foods. If the condition of the food rendered it unfit for human consumption it was condemned, treated, and invariably used as food for animals. There was one seizure of rancid dripping, that resulted in legal proceedings, when a fine of £8 was imposed upon the person responsible for exposing it for sale. The following are the principal articles of unsound food dealt with during the year :—Meat, bacon, margarine, fish, cheese, butter, etc. COLLECTION OF HOUSE REFUSE. The refuse of the Borough has been collected with regularity. This work engages 1G horses and vans (the method being a very slow and expensive one). The Committee realized this some time since, and had it not been for the war, these slow moving horses would have been replaced with electrically propelled dust vans. 40 It is hoped that when the next Annual Report is written it will be able to refer to the horses as relics of the past. We are anxiously awaiting the delivery of a fleet of six electric dust vans, which will, we confidently expect, effect a considerable saving in the cost of work, and provide a more efficient method of collection. Trade refuse is collected twice weekly from shops in the Borough. Special vans are allocated to this work, and 'during the year 1,143 loads have been dealt with. WASTE MATERIAL SALVED FROM THE REFUSE. In the spring of the year a scheme was adopted by the Council sanctioning the salvaging from the dust, material with a market value. The scheme provided that 50 per cent. of the profits should be given to the men. At the time this work was commenced prices obtainable for goods were very high, but when the war ceased prices dropped, as did the amount of material salved. ENFORCEMENT OF THE FOOD CONTROL ORDERS. The Sanitary Inspectors have, during the period of control, carried out the work of enforcement, and during the year 1918 were responsible for bringing 53 cases before the Court for contravention of the various orders made by the Food Controller. The fines and costs resulting from these cases amount to £414 7s. 6d. The offences chiefly comprised overcharging, giving short weight or measure, or imposing a condition of sale. The articles principally affected were meat, whisky, beer, milk, potatoes, flour, and jam. In many instances the price charged was correct, but either the weight or measurement was short, thus increasing the price of the commodity. This applies principally to milk and spirits. Some of the oases were for selling beer below the specific gravity provided by the Order. In many of the meat cases, the prosecution was for charging above the maximum price, and in isolated oases wrong descriptions were given. In one case that was dismissed the butcher was summoned fo selling a middle rib and charging wing rib price. The Stipendiary, after viewing in Court the different ribs of 48 beef, and seeing them cut into different shapes, gave the defendant the benefit of the doubt. Shortly after the hearing of this case, the Ministry of Food issued a new schedule of meat prices, in which it was defined what was to be sold as wing rib. SALE OF FOOD AND DRUGS ACT. There have been 334 samples taken during the year, of whiich 34 were found to be adulterated, these being equal to 10.2 per cent. of samples. It will be see from the table that 253 samples of milk were taken, and 30 were found to be adulterated. This is equal to 11.4 per cent. of the samples. Milk always holds the premier position among the articles subject to adulteration. Since the passing of the Rat Order, 1918, made by the Food Controller, the Council have given special attention to the work of eradicating these pests from the Borough. A professional ratcatcher has been engaged, and contracts entered into, with excellent results. The infected areas have been the centre of attention, and 708 rats have been caught in cages, but undoubtedly large numbers have been exterminated by the virus which is laid in their tracks. It is essential that the work should continue, for owing to the juxtaposition of the docks to the Borough, there will always be the danger of invasion by these pests. The damage to property, food and general depredation wrought, is sufficient to warrant the expense incurred. It has been established beyond doubt that rats are the disseminators of certain diseases that affect human beings. Diseases (e.g., Bubonic Plague), are transmitted from rat to rat, and from rat to man through the rat flea. It is difficult to deal with rats when they invade a dwellinghouse, but it can be claimed that the efforts made in several instances have succeeded. CONTAGIOUS DISEASES OF ANIMALS ACTS. No case of Swine Fever has been reported in the Borough during the year. The provisions of the Swine Fever (Movement) Order, 1908, have been carefully carried out, 18 licences having been received authorising the bringing of 163 pigs into the Borough for killing purposes. 25 licences have been issued upon the receipt of declarations, granting permission for 416 store pigs to be brought in and fattened for food. Two cases of Parasitic Mange have been notified and dealt with according to the requirements of the Parasitic Mange Order, 1918, and the premises have been cleansed and disinfected, and precautions taken to stop the spread of the disease. CINEMAS AND MUSIC HALLS. The premises licensed for music, dancing, and cinema performances have been regularly inspected, and special attention given to overcrowding, and especially seeing that staircases and 54 passages were kept clear and free from obstacles. In several instances it was found necessary to draw the attention of managers to slight breaches of the Cinema Byelaws, which received attention. In no case was it necessary to have recourse to legal proceedings. PETROLEUM AND EXPLOSIVES ACTS. The provisions of the above Acts have been insisted upon w here licences have been granted. It has been found necessary in a few instances to call attention to minor infringements, which are usually t'he result of not knowing the requirements of the Acts governing these articles. There are some 22 premises licensed for petroleum, and 39 for the storage of explosives. PUBLIC ABATTOIRS. For many years in my Annual Report, I have urged the closing of private slaughterhouses and the establishment of a Public Abattoir. To give this suggestion practical effect is a problem difficult to solve. There are sixteen private slaughterhouses in the Borough. Some are well managed, while others have been a serious menace to the suppression of the imposure of diseased meat on the public. A special report dealing fully with The question of closing private slaughterhouses and the provision of a public abattoir for the Borough was presented to the Public Health Committee, who inspected abattoirs near London, and after carefully considering the question from various aspects decided to defer the further consideration of this question until post-war conditions were more satisfactorily established.
